Steps for completing the Survey Online
Step 1: Visit www.tellbanfield.com and enter the valid 19-digit survey code printed on your invoice.
Step 2: Respond to all questions on the On-line survey and provide the requested information include your age and day time phone number.
Step 3: The end of survey you will be entered into the Banfield survey quarterly sweepstakes.

The sweepstakes is open to legal residents of the 50 United States and District of Columbia, U.S. Participants who are aged 18 years & over who have access to the internet.
